目的 探讨疲劳与倦怠的关系,及满意度在二者关系中的调节作用,为基层医务人员倦怠的干预提供一定理论 依据和指导。方法 采用多阶段分层抽样方法,对广州、江门、惠州、潮州 4 个样本市的 622 名基层医务人员进行调查,调查 内容包括调查对象的基本情况,采用疲劳量表、满意度量表、工作倦怠量表进行调查。最终回收有效问卷 533 份。将调查数 据运用 SPSS 22.0 和 AMOS22.0 进行整理与分析。结果 医务人员的疲劳对情绪衰竭(23.1%)和人格解体(7.3%)具有正向预 测作用,而满意度对疲劳与倦怠间的关系有负向调节作用 (β= -0.096; β= -0.130, 0.01)。结论 我国基层医务人员的职业倦 怠较为普遍,基层医疗机构的管理者应从减少医务人员疲劳和提高满意度两方面入手对医务人员的职业倦怠进行干预。通过 增加工作资源,提供组织支持,来提高医务人员的满意度,调节疲劳对倦怠的正向预测作用 , 达到减少疲劳 , 控制倦怠的目的。

英文摘要:

Objective To explore the relationship between fatigue and burnout and investigate the role of satisfaction in alleviating these negative moods among medical staff at the grassroots medical institutions. Methods A survey was carried out in 622 grass-roots medical staff in four sample cities including Guangzhou, Jiangmen, Huizhou, and Chaozhou using a multi-stage stratified sampling method. The survey content included the demographic data and the scores obtained from the fatigue scale, satisfaction scale, and job burnout scale. A total of 533 eligible responses were collected, and the data were collated and analyzed using the SPSS 22.0 and AMOS 22.0 software. Results Fatigue had a positive predictive effect on emotional failure (23.1%) and personality disintegration (7.3%), whereas satisfaction negatively regulated the fatigue and job burnout (β= -0.096; β= -0.130, P<0.01). Conclusion Job burnout is common among grass-roots medical staff in China. The managers of grass-roots medical institutions should intervene the influencing factors in the job burnout of medical staff from the aspects of reducing fatigue and improving satisfaction. Increasing work resources and providing organizational support may help to increase the satisfaction of medical staff and adjust the positive predictive effect of fatigue on burnout.
