摘要:

目的 提高医护人员对患者转交接制度的依从性,提升院内患者转交接规范性,保障患者安全。方法 分析院内转交接依从性差的原因,应用ISBAR 结构化模式制定针对性措施进行质量改进,以提高转交接工作的同质化与规范性。结果 通过质量改进,护理人员对患者转交接工作的依从性增加,医院内患者转交接制度得以规范执行。结论 ISBAR 结构化模式是一种有效的患者转交接工具,本地化、信息化、跨部门合作以及全面持续的质量管理是落实ISBAR 应用的有效保障。

英文摘要:

Objective To increase the compliance of medical staff with the patient handover policies and promote the standardization of patient handover in the hospital, thus ensuring patient safety. Methods The reasons for poor compliance of handovers were analyzed, and the targeted measures in the ISBAR structured model were applied to improve the homogeneity and standardization of patient handovers. Results After the implementation of these measures, the compliance of nursing staff with the handover practices was increased, and the patient handovers in the hospital have been standardized. Conclusions The ISBAR structured model is an effective patient referral and handover tool. Localization, informatization, cross-departmental cooperation, and comprehensive and continuous quality control are essential for the implementation of ISBAR applications.
